Location: br/br/Canton of Neuchâtel (area assigned according to your place of residence) Activity rate: br/br/60 to 100% Start date: br/br/As soon as possiblebr/br/Duration of assignment: br/br/Until the end of Septemberbr/br/For one of our clients, a home care organisation active throughout the Canton of Neuchâtel, we are looking for severalbr/br/Assistants in Care and Community Health (ASSC)br/br/to strengthen the teams. br/br/Responsibilities Provide basic care and delegated medical-technical care at patients' homes.
Support beneficiaries in daily life activities while promoting their autonomy.
Observe patients' health status and communicate relevant information to the multidisciplinary team.
Participate in the development, monitoring and evaluation of care plans.
Ensure individualised, quality care that respects patients' needs.
Ensure care traceability and comply with current protocols.
Collaborate with various health professionals involved in home care. br/br/Profile Holder of abr/br/CFC as Assistant in Care and Community Health (ASSC)br/br/or a recognised foreign diploma (or in the process of recognition) allowing professional practice in Switzerland.
Experience in home care is an asset.
Autonomous, organised, flexible and with excellent interpersonal skills.
Ability to work independently while collaborating effectively with a multidisciplinary team.
Good command of French, both spoken and written.
Category B driving licence mandatory.
