Somatic Nurse Referent (60-100%)
Your mission
As a somatic nurse referent, you take on the role of case manager. You perform and delegate quality services to ensure the health, safety and well-being of clients at home. In their living environment, you define with them the most suitable solutions for their situation, thereby developing a privileged relationship of trust.

Your main responsibilities
- Assess the clinical needs of the client and build their personalised care plan in partnership with them and their relatives - Visit clients and provide the services planned in the intervention plan. - Ensure network management and collaborate closely with care and home help staff while ensuring the quality of services. - Document follow-ups in the electronic file according to institutional guidelines, thus guaranteeing continuity of care. - Create and maintain lasting links with partners in the regional medico-social network (professionals, volunteers, user groups). - Fulfil the referent role in the overall follow-up of cases: assessment, orientation, advice, support with administrative procedures.
Your profile:
- Bachelor of Science in Nursing (HES) or equivalent qualification (recognition by the Red Cross required) - Good knowledge of the Swiss socio-health sector (an asset) - Excellent analytical, synthesis and communication skills. - Fluency in French (spoken and written) and strong writing skills. - Ease in priority management and taking initiative. - Proficiency with common computer tools. - Team spirit, service orientation and ability to work autonomously as well as in interdisciplinary collaboration.
We offer
- Mainly daytime hours (7:00–17:00), with only one evening shift every two weeks until 22:00 and one weekend shift every eight weeks. - Working conditions and remuneration according to the collective labour agreement of the Vaud parapublic health sector. - A quality occupational pension scheme, beyond legal minima. - 5 weeks of holiday per year, then 6 weeks from age 50. - Opportunities for continuing education and career development within the AVASAD network. - A stimulating and humanly rich professional environment, dedicated to the well-being of clients.
