Missions The POOL-EMRAD (Mobile Home Return Team) has the missions to: Take charge of patients at home during the first days after hospital discharge Ensure nursing replacements and extended hours coverage within the East Vaud CMS. Provide nursing care in the
evenings and 2 weekends per month
for the East Vaud CMS. Coordinate care during weekends and public holidays (for those with the necessary qualities for this support and guidance role on the ground). - After a period of integration and specific training on the diversity of situations and the CMS population, you will be autonomous in managing nursing care. - Supported and surrounded by an expert multidisciplinary team, you perform medical-technical care and ensure the follow-up of our beneficiaries. - You develop a care plan with the beneficiary, taking into account their priority needs, resources, those of their entourage, and the Mobile Home Return Team. - You mobilise internal and external resources, coordinate care, and ensure follow-up of the situation, including by updating computerised records and actively participating in case conferences and team meetings. - Your activities are carried out in close collaboration with the multidisciplinary team, the medico-social network, hospital and/or on-call doctors, as well as partner medico-social institutions. - By favouring a holistic approach to the person, you ensure prevention and health promotion for our beneficiaries and their entourage. Location Offices based in Rennaz, travel within the Riviera-Pays-d'Enhaut and Aigle districts Your profile Bachelor's degree in general nursing, level 2 or HES.
