You will participate in the evolution of the home care and community health sector by meeting the needs of our beneficiaries with us, based on our values: TRANSPARENCY – EQUITY – RESPECT – TRUSTNURSE IN THE EMRAD POOL AT 80%MissionsThe EMRAD POOL (Mobile Team for Return Home) has the following missions:- Take charge of patients at home during the first days after discharge.- Carry out nursing replacements and cover extended hours within the CMS X Est vaudois.- Ensure nursing care in the evening and 2 weekends per month for the CMS X Est vaudois.- Coordinate care during weekends and public holidays (For people with the necessary qualities for this support and guidance function for the field).- After a specific integration and training period regarding the diversity of situations and the organization of the CMS, you will be autonomous in the management of nursing care.- Accompanied and surrounded by an expert multidisciplinary team, you perform medico-technical care and ensure the follow-up of our beneficiaries.- You develop a care project with the beneficiary, taking into account their priority needs, their resources, those of their entourage, and the Mobile Team for Return Home.- You mobilize internal and external resources, coordinate the care, and ensure the follow-up of the file, including by updating records and your active participation in colloquiums and team meetings.- Your activities are carried out in close collaboration with the multidisciplinary team, the medico-social network, hospital and/or on-call doctors, as well as partner medico-social institutions.- By favoring a global approach to the person, you ensure prevention and health promotion for our beneficiaries and their entourage.LocationOffices based in Rennaz, travel in the Riviera-Pays-d'Enhaut and Aigle districts.Your profile- Bachelor of Nursing in general care, level 2 or HES.
