ASPMAD, IS: - 9 CMS in Nord vaudois - 950 engaged employees - More than people supported each year - A presence in 105 municipalities Referral Nurse Yverdon-Ouest 80% FOR OUR CMS IN YVERDON-OUEST, WE ARE RECRUITING: REFERRAL NURSE AT 80% YOUR MISSION As a referral nurse, your role involves advanced mastery of personalized specific competencies: Propose, organize and coordinate intervention plans tailored to clients, ensuring their health and strict compliance with respect.
Supervise and regularly re-evaluate actions to ensure safety and continuity of care.
Document management: Responsible for managing OPAS sheets and updating client records in accordance with interdisciplinary collaboration: Work closely with the entire network (CMS team, caregivers, doctors, partners) to ensure appropriate care.
PROFILE You hold a HES nursing diploma or an equivalent qualification, reinforced by 2 years of experience, preferably in home care.
You are versatile, attentive, and autonomous; possess excellent interpersonal skills and show a strong interest in home care.
Your ability to anticipate needs and adapt to any type of situation and your competence in stress management are qualities you possess.
You hold a driver's license and ideally have access to a personal vehicle.
