$85,126 – $91,597 per year + Super and Salary Sac

About the role – Part-time (60.8 hrs per fortnight), Contract ending 30 June 2025

As a Case Manager within the Aboriginal and Torres Strait Islander Men’s Accommodation Service (ATSIMAS) Programme, you will provide comprehensive assessment, individualised case management, mental health support and purposeful transition planning within a Trauma Informed practice framework with the aim of resolving homelessness and supporting mental health recovery and stability.

An objective of this position is to enhance the quality of client experience and satisfaction of the service, and to promote continuity of care. To achieve this, case management processes are inclusive of the strengths and capacity of individual clients and promote achievable goals in negotiation with housing providers and funding bodies.

Reporting to the Team Leader, duties include but are not limited to:

  • Developing individualised holistic assessments and goals/case plans for clients through a person centred and trauma informed lens to achieve positive outcomes
  • Meet regularly with allocated clients to continuously assess, plan and review client goals and strategies using support plans, case notes and records
  • Engage relevant stakeholders to partner with for various projects, programs, activities, or client support where appropriate
  • Ensure compliance with all organisational and service requirements, including record keeping, data entry, and reporting
  • Acting in accordance with relevant legislations, regulations, standards, and Codes of Conduct
  • Provide respectful support to each client utilising skills in emotional support and debriefing, de-escalation of distress and behaviour management

About You:

On top of having prior experience in case management, you will have a strong connection and passion when it comes to supporting marginalised people experiencing homelessness, addiction, or mental health issues.

You will also bring the following skills and experience:

  • Qualifications in a related discipline (social sciences, health, psychology, social work, community services, management)
  • Expresses a commitment to ongoing development of relationships with Aboriginal and Torres Strait Islander community members
  • A comprehensive understanding of community development principals and appropriate referral services
  • Case management experience and evident capability to individualise services to support people who are disadvantaged or at risk
  • Incredible interpersonal skills to build healthy and supportive relationships with clients
  • Emotional intelligence to apply informed decision making to a diverse and complex working environment

Specific Employment Requirements:

  • Full unrestricted South Australia Driver’s Licence
  • DHS Working with Children Check
  • National Police Clearance (or willing to obtain)
  • First Aid Certificate (or willing to obtain)
  • Safe Environments for Children and Young People (or willing to obtain)
  • Travel within South Australia and interstate required
  • Ability to work outside of normal office hours may be required
