Position Summary

The Clinical Pharmacist supports the delivery, implementation, and maintenance of pharmacy services that promote high-quality pharmaceutical care. This role involves active participation in clinical services within the pharmacist’s scope of practice and a strong commitment to exceptional customer service and patient care.

Key Responsibilities

Medication Order Verification

• Review medication orders for accuracy, appropriateness, dosing, and patient-specific factors (e.g., pediatric, neonatal, geriatric populations).

• Interpret and transcribe orders accurately into the Electronic Medical Record (EMR).

• Clarify and correct incomplete or ambiguous orders with providers.

• Verify IV and all other medication orders for prioritization and dispensing accuracy.

• Prioritize and designate orders for clinical follow-up using clinical surveillance tools.

• Collaborate with prescribers on formulary alternatives when necessary.

Medication Preparation and Dispensing

• Prepare and verify medication packaging (e.g., bottles, capsules, unit-of-use).

• Compound sterile preparations using aseptic technique in accordance with USP 797 standards.

• Prioritize and label preparations based on urgency (STAT: 30 min, NOW: 60 min, others as indicated).

• Review and verify technician-prepared products (e.g., repackaging, barcoding, code trays).

• Maintain proper inventory levels and provide constructive feedback to pharmacy staff.

Clinical Services & Pharmaceutical Care

• Modify, discontinue, or initiate drug therapy under collaborative practice agreements or protocols.

• Recommend and monitor laboratory tests.

• Assess therapeutic responses, patient adherence, and potential drug interactions.

• Provide clinical consultations, in-services, and patient counseling.

• Document interventions and recommendations in the EMR.

• Attend interdisciplinary rounds, antibiotic stewardship reviews, and respond to code/RAT events.

Drug Information & Quality Improvement

• Provide accurate and timely drug information to healthcare staff.

• Assist in policy development, medication usage evaluations, and formulary reviews.

• Participate in adverse drug reaction and medication error reporting.

• Contribute to departmental quality initiatives and compliance with regulatory standards.

• Support and participate in Pharmacy & Therapeutics committee activities.

Supervision, Training & Departmental Support

• Supervise and support pharmacy technicians to ensure workflow continuity.

• Participate in orientation and training of new staff.

• Conduct monthly area inspections and documentation reviews.

• Enter and manage data in hospital systems, including 340B program compliance.

• Initiate and lead special projects and performance improvement initiatives.

Organizational Responsibilities

• Attend and engage in departmental meetings and training.

• Complete all mandatory education and health requirements within specified timeframes.

• Maintain licensure, certification (BCLS/ACLS as required), and regulatory compliance.

• Follow universal precautions and safety protocols.

• Be available for additional shifts or overtime as needed.

Qualifications

Education & Experience

• Doctor of Pharmacy (PharmD), Master’s, or Bachelor’s degree in Pharmacy from an accredited institution.

• Indiana Pharmacist Licensure (or eligibility).

• Two years of hospital pharmacy experience and/or completion of a residency preferred.

• Required: BLS and ACLS certification (full-time); preferred for part-time and PRN.

• If holding a Bachelor’s only, clinical experience or residency is required.
