- Complete medication reconciliation, clinical exams, and chart reviews to identify members needing higher-level care.
- Triage referrals and facilitate handoffs to care team members.
Success Metrics:
- Timely outreach to members and hospital case managers for transitions of care.
- Accurate completion of assessments and medication reconciliation.
- Efficient management of multiple short-term clinical assignments.
Requirements:
- Graduate of an accredited school of nursing (RN) with 3+ years of experience.
- Strong critical thinking and clinical judgment, with ability to work independently.
- Excellent communication skills and comfort with virtual and in-person engagement.
