Job Summary Seeking a qualified NYS Licensed Mental Health Counselor (LMHC), Licensed Clinical Social Worker (LCSW), or Licensed Master of Social Work (LMSW). Hybrid remote position with several locations to choose from.
Responsible for providing psychotherapy, clinical assessment and evaluation to identify and treat disabilities and problems of behavioral, developmental, emotional, or social functioning in individuals served at Finger Lakes Community Health in accordance with the New York State Department of Health Article 28 regulations for a Diagnostic and Treatment Center.
Main Responsibilities
Evaluate and complete assessment; provide verbal talk therapy or behavioral methods to treat disorders of behavior, character, emotional, or personality.
Use assessment instruments in addition to mental health counseling to identify and evaluate disorders and dysfunctions of mental illness.
Provide mental health counseling using evidence-based approaches such as Cognitive Behavioral Therapy, Dialectical Behavioral Therapy, Solution Focused Therapy, and Motivational Interviewing.
Develop and implement an individualized treatment plan that includes diagnosis/assessment, goal, objective, barriers to treatment, strengths of the individual, modality and intervention used, and target date.
Coordinate care with an integrated focus: team-based coordination with primary care; participate in behavioral health case conferences; serve as a core member of a multidisciplinary care team; participate in planned supervision for license and consultation.
Document progress notes and treatment plans in an integrated electronic medical record, including presenting problem, status of symptoms, treatment goals, progress towards goals, and functional status. Documentation may require supervision and co-signature of notes by a Licensed Clinical Social Worker.
Education and Qualifications
A current unrestricted NYS license is required.
Ability to communicate fluently in both Spanish and English.
CPR certification required (can be provided).
Strong interpersonal skills and ability to build relationships with personnel, patients, and community partners.
Proven ability to handle confidential information with discretion.
Ability to work flexible hours—some evenings are required.
Physical Requirements
Must be able to hear and communicate with clients and staff via phone, web media, and in person.
Must be able to lift up to 50 lbs.
Must be able to sit for extended periods of time.
