- Bachelor HES-SO in Nursing or equivalent, or foreign diploma recognised by the Swiss Red Cross - Knowledge of the home care sector - Effective in conducting communication processes adapted to patients and their relatives, as well as professional partners - Able to collaborate within a multidisciplinary team - Listening skills and ability to provide support - Analytical mindset - Ability to be proactive and to position oneself as a partner in networked work - Ability to adapt to change, work autonomously and manage complex and/or crisis situations - Driving licence essential

- RAI-HC method (Resident Assessment Instrument - Home Care) - Knowledge of Medlink software - Interest in mental health
Your future tasks
- Carry out the comprehensive medico-social assessment at the client’s home using the RAI-HC method up to the development of the intervention plan and its transmission to the referent. - Provide care and support adapted to people of all ages, in various contexts, in collaboration with their relatives. - Actively participate in a multidisciplinary manner in the organisation and follow-up of the care project defined with the client.
