To complete the team at the CMS des Peupliers, the Fondation Soins Lausanne is looking for immediately or as agreed three : Coordinating Nurse (M/F) Activity rate from 50% to 100% Permanent contract Your missions Identify and analyse the client's immediate needs and establish the initial intervention plan to optimise continuity of care.
Secure the client's return home and coordinate care during the support period.
Provide care during the support period.
Ensure an on-call telephone service in the evenings, weekends and public holidays.
Collaborate with the client network's stakeholders and respond to their requests.
Intervene if necessary in acute situations at home.
