Your mission
You play the role of clinical expert with clients and their relatives, serving the home life project. Your practice is based on evidence.
Clinical practice and expert reasoning
- Assess and manage complex situations through expert clinical reasoning - Adapt care to the needs, values, and context of the person, with a population health perspective. - Contribute to health promotion and prevention.
Support to teams
- Support teams in complex situations (consultation, guidance, coaching, supervision) - Disseminate best practices and strengthen caregivers’ skills - Supervise clinical documentation and integrate new collaborators
Innovation and change
- Lead continuous improvement and innovation projects - Guide the implementation of new practices and encourage team adherence
Collaboration and coordination
- Promote coordinated teamwork, both intra- and interprofessional - Strengthen collaboration with network partners - Consolidate partnership with the client and their relatives
Evidence and quality
- Ensure interventions based on updated evidence, adapted to the home - Participate in protocols, analysis of adverse events, and monitoring of indicators - Contribute to the development of nursing sciences in home care
Ethics
- Promote ethical reflection in clinical and institutional practices
Your profile and skills
- Bachelor in nursing (or equivalent qualification) - Master’s degree in nursing sciences mandatory - Proven experience in the medico-social field, community health, and clinical practice - Excellent clinical assessment skills and management of complex situations - Demonstrated ability to work in interdisciplinary collaboration, with partners and networks - Sense of responsibility, initiative, and ability to organise autonomously and set priorities - Ability to create a climate of trust and exchange within a team - Confirmed project management skills
