- Title: Special Needs Coordinator
- Location: Geilenkirchen, Germany
- Start Date: Immediately
- Job Type: Part-Time on Call
The Part-Time on-call will cover the position during scheduled absences and sick leave for the full-time staff.
Moving to a Full Time would be available late next year.
JOB DESCRIPTION
- Establish and maintain procedures to identify sponsors whose family members have special medical and educational needs promptly.
- Perform administrative duties concerning the coordination, processing, review, and maintenance of medical records and information about individuals and families assigned to this installation or preparing to serve in overseas assignments and some CONUS assignments.
- Maintain records to ensure service availability for families relocating; process Command Sponsorship requests and documents.
QUALIFICATIONS
- Social worker or nurse.
- Minimum of two (2) years of full-time experience in care coordination, discharge planning, or case management in a medical setting within the last four (4) years.
- Current Basic Life Support (BLS) certification.
- Basic computer skills and working knowledge of Microsoft Office products such as Word, Excel, and Access.
