  • Lead the Connect For Life program using Wider Circle's curriculum to build trust and promote health
  • Lead virtual and in-person events and create connected member communities
  • Plan and host events, including venue search, preparation, and member attendance outreach
  • Proactively contact members, support Chapter events and group activities, conduct one-on-one home visits, and facilitate monthly engagement
  • Meet monthly member engagement, attendance, and care-needs goals
  • Record all member outreach, attendance, and contact attempts in Wider Circle's digital systems
  • Identify community leaders and invite them to become Wider Circle Ambassadors
  • Contact prospective members and develop personalized enrollment plans
  • Connect members with health plans and one another
  • Help members access healthcare services, doctor appointments, and close care gaps
  • Monitor engagement and growth metrics, including attendance and feedback
  • Track and report hours worked
  • Conduct home canvassing and in-home visits when required
  • Drive to public places up to 30 miles from home and carry event supplies

Requirements

  • 3+ years in community outreach, teaching, or facilitation strongly preferred
  • Healthcare experience is a plus
  • Associate's degree preferred; 5+ years of relevant experience may be considered instead of a degree
  • Experience creating or supporting a community of 500+ members
  • Able to prioritize tasks, stay organized, and follow through on commitments
  • Comfortable using web-based and app-based systems
  • Able to manage tasks and responsibilities in a remote environment with little supervision
  • Reliable transportation for home visits; willing to travel 20-50 miles
  • Available to work outside regular business hours when needed
  • Willingness to complete pre-employment drug testing and background checks
  • Bilingual (English/Spanish) is a plus
  • Excellent communication and presentation skills
  • Outgoing, positive, and able to engage people in person and virtually
  • Strong networking and leadership skills, with experience guiding peers
  • Able to work well with others, contribute to a positive environment, and provide leadership when needed
  • Take the lead on community engagement initiatives with minimal direction
  • Comfortable in a fast-paced, changing environment, and eager to learn and grow
  • Safely and consistently drive to public places up to 30 miles away from home
  • Frequently carry up to 30 pounds of supplies
  • Frequently stand and comfortably speak publicly, including projecting your voice in indoor and outdoor spaces
  • Routinely sit, stand, and walk throughout events
  • Ability to see and hear well enough to interact with members by phone and in person
  • Ability to use a computer to view, enter, and update information in different systems
  • Effectively communicate with members in person or virtually
