- Conduct comprehensive evaluations of patients' physical, cognitive, and psychosocial functioning
- Collaborate with clients, families, and healthcare professionals to establish therapy goals
- Design and implement individualized treatment plans
- Provide therapeutic interventions to improve independence in activities of daily living
- Use evidence-based techniques, adaptive equipment, and assistive devices
- Educate clients, families, and caregivers on therapeutic techniques and safety measures
- Coordinate holistic care with physicians, nurses, COTAs, and social workers
- Maintain documentation of evaluations, treatment plans, progress notes, and discharge summaries
- Stay updated through continuing education and professional development
- Adhere to ethical and professional standards, confidentiality, and respectful care
Requirements
- Minimum one year experience in providing occupational therapy services in an adult home health setting
- Bachelor's or Master's degree in Occupational Therapy from an accredited program
- Texas State License as an Occupational Therapist
- Knowledge of Kinnser/Wellsky EHR
- Experience with communication apps such as Slack
- Valid CPR BLS certification
- Ability to perform comprehensive assessments, develop treatment plans, and deliver evidence-based interventions based on individual clients' needs
- Excellent interpersonal and communication skills
- Bilingual English/Spanish preferred or advantageous
