- Lead clinical strategy at scale for post-acute care
- Guide the design, launch, and expansion of specialty consult services across multiple states
- Work with physician specialists, executive leaders, NPs, PAs, and facility teams
- Define clinical protocols
- Oversee quality metrics
- Mentor providers and develop advanced practice providers
- Manage multi-site clinical operations

Requirements
- Master of Science in Nursing (MSN)
- Active Nurse Practitioner license in good standing (multi-state preferred)
- 10+ years of NP experience in PALTC, SNF, or LTACH settings
- 5+ years of experience in leadership, education, or program development
- Clinical background in cardiology, pulmonology, nephrology, or infectious disease
- Proven success building and leading NPP teams
- Strong knowledge of CMS regulations, documentation standards, and value-based care
- Proficiency with EHR systems, dashboards, and care coordination platforms
- Ability to operate at scale while empowering clinicians on the ground
- Doctor of Nursing Practice (DNP) strongly preferred
