- Perform telephone triage using established protocols and sound nursing judgment
- Assess symptoms, prioritize patient needs, and determine appropriate care or follow-up
- Provide patient education, guidance, and reassurance
- Document patient encounters in Epic EMR in real time
- Escalate urgent or emergent situations according to clinic and organizational policies
- Communicate with on-call providers, clinic teams, and patients to ensure continuity of care
- Contribute to safety, quality, and patient satisfaction within the triage team
- Utilize the nursing process to assess, diagnose, plan, implement, and evaluate individualized care
- Recognize changes in patient status and modify or modify the plan of care
- Advocate for patients and support informed consent and self-determination
- Participate in practice governance, quality improvement, research, and evidence-based practice
- Manage relationships, mentor colleagues, and delegate care appropriately
- Prioritize and organize time to optimize patient outcomes
- Communicate effectively across practice formats and partner with others to effect positive change
- Precept nursing students, residents, experienced clinical nurses, and healthcare team members
- Perform other duties as required or assigned
Requirements
- Active, unrestricted RN license; multi-state compact license required
- Minimum 2–3 years of clinical experience with a strong background in triage, ER, Med-Surg, and/or Primary Care
- Proficiency in Epic EMR and remote communication systems
- Excellent assessment, critical thinking, and decision-making skills
- Strong communication, documentation, and time management abilities
- Self-motivated and comfortable working independently in a remote environment
- Associate degree in nursing required; BSN preferred
- Previous experience in remote triage, nurse advice lines, or outpatient/clinic settings preferred
- Reliable internet and a secure HIPAA-compliant workspace, free from distractions
- Graduate from an accredited Nursing Program required
- Licensed Registered Nurse in New Hampshire required
- Basic Life Support (BLS) Certificate required upon hire
- Eligible candidates must reside in Arizona, Colorado, Connecticut, Florida, Georgia, Indiana, Kentucky, Maine, Maryland, New Hampshire, North Carolina, Pennsylvania, Rhode Island, South Carolina, Tennessee, Texas, Utah, Vermont, or Virginia
