- Identify, oversee, and manage coordination of member transitions in the community
- Ensure timely and safe transitions between various levels of health care services
- Coordinate care plans with community care coordinators
- Educate transition enrollees about services, requirements, limitations, and exclusions
- Support care coordination teams, providers, and healthcare team members in developing transition plans
- Assist with transitions based on enrollment or transition-of-care services
- Identify new member enrollees requiring transition services
- Ensure existing authorizations are honored during transitions
- Serve as a resource for members, families, and caregivers
- Track and maintain transition metrics and document transition-of-care information
- Draft educational materials and resources
- Assist with developing education and training programs
- Evaluate member needs and available resources and recommend and facilitate care plans
- Coordinate between members, caregivers, and care provider teams
- Interact with healthcare providers to facilitate care coordination
- Perform other assigned duties and comply with policies and standards
Requirements
- Bachelor's degree required
- 2-4 years of related experience
- Equivalent experience may be acquired through accomplishments of applicable knowledge, duties, scope and skill reflective of the level of this position
- Background in healthcare and/or behavioral health
- Familiarity with Wisconsin Medicaid
- Ability to manage a high volume of inbound and outbound calls
- Ability to complete assessments and develop individualized care plans
- Availability Monday to Friday, 8-5 pm CST
