The Early Action and Support Officer plays a key role in leading, developing and improving the Adult Care and Community Wellbeing (ACCW) prevention offer.

This role helps ensure that residents, carers, staff and partners can access clear, accurate, timely and accessible information that supports independence, wellbeing, informed decision-making and early access to the right community-based support.

This is a varied and influential role, combining service improvement, digital content development, partnership working, team supervision and performance monitoring.

The postholder will work with internal teams, external partners, community organisations and wider system stakeholders to strengthen the consistency, quality and reach of preventative services and information and advice across Lincolnshire, with a focus on supporting resilience and helping people remain independent for longer.

This post is offered as a fixed term position or secondment (internal) for a year.

Please note however, that the role has funding for two years so there is the possibility of contract extension.

The team is based on the Lincoln County Offices campus but there is opportunity to work from home or flexibly at other satellite offices across the county.

  • What you will be doing
  • Leading the development, promotion and continuous improvement of preventative services and information and advice provision across Adult Care and Community Wellbeing.
  • Supporting colleagues, providers and partners to deliver high-quality, consistent and accessible information that helps residents and carers understand their options, act early and draw on community support where appropriate.
  • Working with communities, voluntary and community sector partners and wider stakeholders to identify local assets, strengthen community capacity and promote inclusive, preventative approaches.
  • Contributing to approaches that prevent, reduce or delay the need for formal care and support by improving access to early help, practical advice, peer support and community-based opportunities.
  • Overseeing digital content, website information, social media assets, newsletters, campaigns and other communication materials that promote independence, wellbeing and community resilience.
  • Producing clear reports, guidance, briefings and presentations to support understanding, decision-making and best practice.
  • Line managing Early Action and Support staff, including day-to-day guidance, supervision, performance monitoring and support for professional development.
  • Building positive working relationships with internal teams, strategic partners, providers, community groups and wider system stakeholders.
  • Monitoring performance, quality and impact, identifying opportunities to improve service delivery, address gaps, reduce avoidable demand, and make best use of resources.
  • Promoting good practice in data protection, freedom of information, safeguarding, equality, accessibility and customer-focused service delivery.
  • Supporting the transition of Adult Social Care in Lincolnshire to the target operating model as outlined in the Adult Social Care Strategy.

We are looking for someone with experience in social care, health, community development or a related public service environment, and a strong understanding of how effective prevention and information and advice can help people to make informed choices, maintain independence and access support before needs grow.

  • You will bring strong organisational, analytical and problem-solving skills, with the ability to plan work, manage competing priorities and support others through change.

Experience of project management, line management, performance monitoring and the use of Microsoft 365 will be important, alongside a commitment to equality, inclusion, safeguarding, strengths-based practice, community resilience and continuous improvement.

In accordance with Part 7 of the Immigration Act 2016 (Fluency Duty), the ability to converse at ease with customers and provide advice in accurate spoken English is essential for the post.
