Proxi-Soins is a home care organisation active in the cantons of Vaud and Fribourg. To strengthen our coordination team, we are looking for a Coordinator Nurse at 80–100% for our Moudon site.
You will play a central role in organising care, supervising teams, and developing the quality of services, in close collaboration with management.
Your main tasks
Care coordination
- Ensure follow-up of new patients and organise their care management. - Supervise care records and ensure their compliance. - Organise and adjust the schedules of field teams. - Ensure continuity and quality of services provided to patients. - Collaborate with doctors, hospitals, nursing homes, and health network partners.
Team management
- Supervise and support nurses, ASSC, auxiliaries, and administrative staff. - Participate in recruitment, integration, and monitoring of employees. - Organise replacements, manage absences and holidays. - Contribute to the development of team skills through support and internal training.
Administrative management
- Supervise administrative procedures related to care services. - Ensure follow-up of assessments, OPAS, and documents necessary for care management. - Guarantee the quality and reliability of data entered in patient records.
Quality and development
- Participate in the continuous improvement of services. - Contribute to updating internal procedures and protocols. - Participate in the development of the structure and its outreach within the care network.
On-call duties
- Participation in telephone on-call duties according to a defined rota. - Coordination of urgent situations and remote support to teams. - On-call duties are mainly carried out remotely, without systematic on-site intervention.
Your profile
- HES nursing diploma or qualification recognised by the Swiss Red Cross. - Professional experience in home care or a similar field. - Experience in coordination, team management, or strong motivation to evolve into this role. - Organisational skills and excellent priority management abilities. - Ease in interpersonal relations and communication. - Initiative, autonomy, and decision-making ability. - Good command of IT tools. - Driving licence essential.
