About This Role
As a Registered Nurse Home Health Baylor, you will guide patients and families through the care continuum by assessing needs, coordinating care, supporting discharge and transition planning, and managing ongoing outpatient care.

What You'll Do
- Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
- Make referrals to other disciplines as indicated by the patient's identified needs or document rationale for not doing so.
- Promote patient health and independence through teaching and appropriate rehabilitative measures.
- Assist patients in learning appropriate self-care techniques.
- Supervise LPNs and HHAs.
- Complete documentation timely and accurately.
- Regularly communicate patient progress to the clinical manager and care team.
- Plan and provide staff education.
- Perform on-call responsibilities and on-call services to patients/families as assigned.
- Participate in clinical development and continuing education programs.
Requirements
- Education:
- Current RN license, specific to the state(s) you are assigned to work.
- Experience:
- 1+ years of clinical experience as a Registered Nurse (RN).
- Licenses & Certifications:
- Valid driver's license, reliable transportation, and liability insurance required.
- Current CPR certification required.
