Pay Rate:Range $1800 to $2000 Weekly Gross pay (Non-taxabletravel/housing and M&I stipend available for qualified/eligible travelers)
MileageReimbursement: $0.50/mile for distancecovered between patient home appointments
Unit: Home Health
Shift: 8 Hours; Days, 8a – 5p
Schedule: 40 hours / week guaranteed, 5 shifts per week, Mon-Fri, No weekends or On-call
Position Requirements:
- Registered Nurse: 1 year - Required
- Recent/Current HomeHealth Nursing experience: 1 year – Required
- Recent/CurrentHospice Nursing experience: 1 year – Required
- Home Care Home Base(HCHB) charting experience – Required
- Must have validDriver’s License and Personal Auto Insurance
- Must be CovidVaccinated or provide medical or religious exemption
Education, Licenseand Certification Requirements:
- ASN or an equivalent– Required
- BSN or an equivalent– Highly preferred
- RN License for Stateof WI or Compact - Required
- BLS Certificationfrom AHA or American Red Cross - Required
Job Description:
- Provide andcoordinate care for assigned patients in comfort of their homes.
- Develop and implementnursing care plans including Start of Care and admissions as assigned.
- Support patientrecovery and ability to stay at home - enabling them to spend quality timewith their loved ones and doing the activities they enjoy.
- Empower patients andfamilies to reach healthcare goals by educating them on disease management.
- Provide Hospice nursingcare and services within the standards and scope of nursing practice asdefined within the state Nurse Practice Act regulations, policies, procedures,and protocols, as well as facility and corporate policies and procedures.
- Ensure physicians’orders are legible if taken verbally and transcribed appropriately perpolicy and procedure.
- Ensure patients’status is assessed on an ongoing basis and pertinent information isgathered and documented.
- Notify physicians ofsignificant changes in status.
- Document and completecharting for all patient visits within 24 hours.
- Provide bereavementsupport to patient’s families as needed.
